Prepare for Your First Visit

This first visit is a consultation between you and your doctor. This important first meeting establishes the right course of treatment based on your condition. Your doctor will perform an oral evaluation, review or order X-rays or 3D imaging, and discuss your options for anesthesia or sedation. Your doctor will also make sure that you are aware of your options for treatment and understand your treatment plan, as well as answer any questions you may have.

Please arrive at your appointment a few minutes early to check in and fill out any remaining paperwork. If you’re a new patient, you may submit your Patient Intake form online.

Important Documents

Please bring these items with you:

  • Referral slip
  • X-rays or other 3D imaging, if available
  • List of current medications
  • List of known allergies
  • Dental and/or medical ID cards
  • Photo ID, such as a driver’s license

Note to Parents of Minor Patients

All patients under the age of 18 must be accompanied to their first appointment by a parent or legal guardian.
