Identifying Oral Abnormalities

Oral surgeons receive years of training to understand, identify, and treat conditions relating to the mouth, salivary glands, and jaws. When you come to Carolinas Center for Oral & Facial Surgery for an evaluation, one of our specialists will provide a comprehensive exam and determine the next steps for treatment. The mucosa is the smooth, pink-colored mucus membrane that lines the inside of the mouth. It is a common area for oral abnormalities to occur.

Abnormalities may include:

Canker Sores

A small, painful ulcer that typically forms on the inside of the mouth. These sores can appear on the inner cheeks, lips, tongue, soft palate, or the base of the gums.

Fever Blisters

Fever blisters, also known as cold sores, are small, fluid-filled blisters that typically appear on or around the lips.

Mucoceles

A benign, fluid-filled swelling that typically forms inside the mouth, commonly on the inner lip, floor of the mouth, or tongue. It occurs when a salivary gland duct is damaged or blocked, accumulating saliva in the surrounding tissue.

Fibromas

A benign (noncancerous) growth of fibrous connective tissue. It commonly appears inside the mouth due to chronic irritation or trauma, such as biting the cheek, lip, or tongue, or from friction caused by dental appliances like braces or dentures.

Ulcers

An open sore that forms on the mucous membrane inside the mouth. These ulcers can occur on the inner cheeks, lips, gums, tongue, or roof of the mouth and are typically painful, especially when eating, drinking, or speaking. 

Cysts

A cyst is a sac-like structure filled with fluid, semi-solid material, or air. They are typically noncancerous but can cause discomfort or complications if left untreated. 

Tumors

A tumor is an abnormal mass of tissue that can be benign (noncancerous) or malignant (cancerous). Oral tumors can develop in any part of the mouth, including the lips, tongue, gums, or palate.

Oral Cancer

A type of cancer that develops in the tissues of the mouth (oral cavity) or the oropharynx (the part of the throat just behind the mouth). It typically begins as abnormal, uncontrolled growth of cells that can invade surrounding tissues and potentially spread (metastasize) to other parts of the body.

Be Proactive with Your Oral Health

It is a good idea to perform a monthly exam of your mouth to familiarize yourself with how your mouth typically looks and feels and to allow you to notice any abnormalities early. When you see a specialist for an evaluation, they will want to know how long you have been experiencing symptoms. 

Changes that can occur inside the mouth may include:  

  • Red or white patches  
  • Sore throat  
  • Chronic hoarseness  
  • Difficulty chewing or swallowing  
  • A sore that does not heal  
  • A painful sore 

Some abnormalities may require further evaluation with a biopsy. A biopsy is a small tissue sample of the affected area that a pathologist evaluates. The pathology information will help your oral surgeon determine the next steps for treatment. Your oral surgeon will determine if a biopsy is recommended. 

Types of Anesthesia for a Biopsy

Local Anesthesia

Local anesthesia numbs a specific area where it is applied without affecting the patient’s awareness. It allows the patient to remain awake while eliminating sensations of pain or discomfort. This type of anesthesia is commonly used for minor medical procedures.

Nitrous Oxide

Nitrous oxide, commonly known as laughing gas, is a mild sedative that patients inhale to help reduce anxiety. It does not cause unconsciousness and is often used alongside local anesthesia for enhanced comfort during procedures.

IV Sedation

IV sedation is commonly used for moderate to complex surgical procedures involving the mouth or face. The sedative is administered directly into the patient’s bloodstream. A typical side effect of this sedation is amnesia, causing many patients to have no memory of the procedure afterward.
